Newly reappointed manager the Ireland coach has included captain Caelan Doris in his Irish squad for the autumn Tests, however lock McCarthy is absent.
The captain, absent from play since undergoing shoulder surgery in the start of May, has come back to provincial training, but it remains unclear whether he will be ready to play against the All Blacks in Chicago on 1 November.
Second row McCarthy, on the other hand, did not make the cut after failing to recover from the foot injury suffered during the first Lions match against the Wallabies in July.
Northern province forward Cormac Izuchukwu and Munster winger Nash also miss out due to fitness concerns, and number 15 Hugo Keenan was previously excluded after having a hip operation after the Lions tour.
Leinster forward McCarthy - Joe's younger brother - is the sole new player in the 34-player squad, however Irish rugby authorities states uncapped Munster pair Edwin Edogbo and Gleeson and previously capped Connacht winger Bolton will travel to the US as "cover".
After facing the New Zealand at Soldier Field in a rematch of their previous encounter, the team will play the Brave Blossoms, the Wallabies and world title holders the Springboks in the Irish capital.

In a largely experienced selection, Leinster's Sam Prendergast and Munster's Jack Crowley will again compete for the first-choice number 10 spot. The number 10 jersey is remains available, as the two players set to face off for the first time in the upcoming URC game at Croke Park.
Ciaran Frawley is the additional fly-half chosen as the coach deciding not to selecting the province's Harry Byrne.
At number 9, the Ireland boss has selected Lions tourist Jamison Gibson-Park and Casey, who captained Ireland to the summer international victories over Georgia and Portugal. There is also a recall for Connacht's Blade, who is preferred to teammate Murphy.
Robbie Henshaw, Bundee Aki, Ringrose and McCloskey represent an experienced group of midfield options. Jamie Osborne is a further choice in the centres, but the youngster is also a frontrunner to cover for the full-back at number 15.
Wingers James Lowe, Hansen, O'Brien and Jacob Stockdale are included. O'Brien impressed on his first Ireland outings over the summer with a brace each in the wins over the Lelos and the Lobos.
McCarthy is the only new name in the Irish forwards. The loose-head prop has been in two of Leinster's three league games this season and featured in the second string's defeat by the English in Bristol previously.
McCarthy was included in interim boss Paul O'Connell's summer squad, yet did not earn a cap.
Munster lock Ahern, who earned his first two caps in those Tests, is retained, while experienced Ulster second row Iain Henderson is back after missing the summer matches.
Jack Conan, who played in all three Lions matches during the tour, is poised to cover for Doris should the skipper fails to recover full fitness soon enough to play the All Blacks.
The national team will meet in Dublin on the start of the week before flying to the US on Tuesday.
